Keep your muscle on weight loss jabs.
Weight loss jabs help many people lose weight, but not all of the weight lost is fat. In research, part of it was lean mass, which includes muscle. Strength training and enough protein are the two things in your hands that help you keep it, and a few simple tests tell you whether you are.
Mostly fat, some muscle.
In the body scan sub-studies of the large weight loss jab trials, people lost a lot of fat, and some lean mass with it:
- In one trial, about three quarters of the weight lost was fat and about a quarter was lean mass. That is a similar share to weight lost by eating less without medication.
- In another, lean mass fell by about a tenth from where it started. In that trial, about 60% of the weight lost was fat and 40% lean mass.
Lean mass is not only muscle; it includes water and organs too. But muscle is the part you can protect, and it matters more with every decade: it is what gets you up from the floor, up the stairs and through a long day.
NICE guidance says these medicines should be used alongside a reduced calorie diet and more physical activity, not instead of them. The British Heart Foundation suggests regular exercise, including resistance training, to help keep muscle while you lose weight.

Five things, in your hands.
- 1. Strength train at least twice a week. Whole body, the big movements: a squat or sit to stand, a push, a pull, a hinge, carrying something heavy. Two short sessions you keep doing beat four you abandon.
- 2. Build in the right order. Core, the back of the body and posture first. Then strength on supported positions. Add weight only when the current weight is steady: more reps than last time, in full control, and feeling easier.
- 3. When the jabs blunt your appetite, protect protein first. Smaller meals make it easy to under eat protein without noticing. Put a source of protein at the centre of every meal before anything else on the plate.
- 4. Measure strength, not just weight. The scale cannot tell fat from muscle. Test the same few things every 12 weeks: push-ups, the sit to rise test, the one leg stand, and your waist to height ratio.
- 5. Talk to your prescriber about your dose, side effects and any health condition before you change how you train. This page is about training, not the medicine.
Four tests, at home.
All four are in the Five Minute Baseline. Do it now and again in 12 weeks: if the weight is coming off and the numbers are holding or rising, you are losing fat and keeping your strength.
- Push-ups. Upper body strength.
- Sit to rise. Strength, mobility and balance together.
- One leg stand. Balance.
- Waist to height. Fat where it matters most.
What people ask first
Do you lose muscle on weight loss jabs?
Some of the weight lost is usually lean mass, which includes muscle. In one large trial’s body scan sub-study it was about a quarter, a similar share to weight lost through diet alone. Strength training and enough protein help you keep as much as you can.
Can you build muscle while on weight loss jabs?
Building a lot of muscle while losing weight is hard at any age. The realistic aim is to keep the muscle you have, and to get stronger while the weight comes off. Strength that rises while weight falls is a good sign.
How often should I strength train on weight loss jabs?
At least twice a week, whole body, building gradually. If you are new to it, start with supported positions and add weight only when a movement feels steady.
Should I eat more protein on weight loss jabs?
With a smaller appetite, many people end up eating less protein without meaning to. Make protein the centre of each meal first. If you have a kidney condition or another health condition, ask your GP or prescriber what is right for you.
